This agreement
between ____________ and Quality Refrigeration
Concepts Inc. states the equipment listed below
will be maintained every ____ days as
suggested by manufacturer. This agreement will
begin one day after signing of agreement, and
will continue month to month until further
notice or upon cancellation by either party.
Cancellation of this agreement must be presented
in writing and will end on the next monthly
billing cycle. Billing will be due on a net 7
day term unless otherwise listed.
During a routine
preventive maintenance you may have standing
water, chemical odors, dust, dirt, grease, etc.
in the area of the equipment being serviced.
Quality Refrigeration Concepts Inc. will not be
held liable for water damage or bodily injury
associated with the preventive maintenance.
